SRNA Evaluation
Submission Forms
Fill out sections "SRNA" and below. Save for your records and email to napinfo@wakehealth.edu. You must submit a case once each semester. Review information about the case (including post-op disposition, patient outcome, etc.) and bring a copy of your anesthesia record to conference. If you are unable to access the submission form above, you may submit the case as a Word file instead. Include MD, CRNA, SRNA, Date, and synopsis of case of about 400 words or 3/4 length of typed page.
