The amendment 6 (closure) letter has been added to the archived amendment section of the 98308 website.
Reminder from Data Management Center: 98308 Diary Card (Appendix 9)
1. The diary should be filled in completely. When the symptoms are not present, enter a zero; please do not leave blank. Every space should have a number in it. The middle section for the number of pills taken also needs to be completed fully. If the patient misses a dose, please write a zero for the missed dose.
2. We're finding it's hard to read the faxed copy of some people's handwriting for the over-the-counter and antibiotic medications, so please print this information, keeping in mind that we have to be able to read a faxed copy.
