Sites participating in 91105 should send their invoices for the $25 reimbursement for the cost of copying the pre-treatment MRI along with MRI Submission Form (Appendix 15) to Gina Enevold at the Protocol Information Office 2000 W First Street, Suite 401 Winston-Salem, NC 27104.
