Enrollment
If you are interested in learning more about Children’s Choice Learning Center, please contact the center directly at 716-0300 to schedule a tourand learn more about this 5-STAR Center. Once you have made a decision to enroll, you will need to pay a non-refundable registration fee and pick up an enrollment packet containing the following documents:
- Enrollment form
- Health information and authorization form
- Health statement
- Acknowledgment of a special need
- Child care tuition payment authorization form
- Corporate sponsor childcare payroll deduction authorization form
- Corporate sponsor parental agreement
- EFT Authorization Form
- Weekly commitment sheet
- Permission to release information
- Family connections
- Back-up feeding plan for infants
- Positive guidance policy acknowledgment
- NC Daycare Laws & Rules
Once you have completed the enrollment packet, you will need to call the Center to schedule a Parent Intake, which must occur at least two days prior to your child’s first day of attendance. Please bring the enrollment packet completed in full to this intake. This intake will allow the Center to ensure all paperwork is completed, and will also give you an opportunity to learn the Center’s policies and procedures, meet your child’s teachers and ask any questions you may have.
