  • Hours for patients with Health Information needs:
                8:00 am to 5:00 pm (Monday - Friday)
    Please bring a picture ID to help us protect your health information. 
  • Request for medical records requires a signed consent form in order for medical records to be released to any entity.   

  • The length of time it takes to process forms is approximately: 
                 Adult Medicine:            4 - 7 working days
                 OB/GYN:                     4  - 7 working days
                 Pediatrics:                   3 - 5 working days
