Patient Photography
We are available to photograph patients in their room, in your clinic, in our studio or in the OR. We are equipped for all types of photography from intra-oral to full body. We are available Mondays through Fridays from 8 am to 5 pm and after hours by request. Please schedule a photographer in advance by calling 716-4298.
Presentation sized files are delivered via email and publication quality files are archived. Our photographers are HIPAA aware.
How To Get Started
- Who is the attending physician and contact person?
- What is the room number and location?
- What will we be photographing?
- Is a size reference scale required?
- Is there a signed patient release (consent form)? If not, the photographer will have a consent form. Download consent form.
- Prior parental approval is required for any photography of minors.
- A chaperone is required for clinical photographs — is one available?
- We are required to bill for our services. What is the billing account number?
