JOB SUMMARY:

Responsible for the planning, development and daily operations of the ANCHOR program. A Community based Care Transitions Program designed to improve care transitions from the hospital to the home and to reduce readmission for high-risk Medicare beneficiaries. Currently serving patients in Forsyth, Davidson and Surry Counties. Collaborates with community agencies such as Senior Services, Right at Home, Advanced Home Care, Gentiva, and Piedmont Triad Regional Council Area Agency on Aging (AAA).

EDUCATION/EXPERIENCE: Bachelor's degree in Nursing, Healthcare Administration or related field. MSN or Master's preferred. A minimum of 3-4 years of project management experience working with multiple medical disciplines and community groups in a variety of settings.

REPORTS TO:Deputy Director, NCCN

LICENSURE, CERTIFICATION, and/or REGISTRATION: Licensure as appropriate to education.

ESSENTIAL FUNCTIONS:

1. Ensures that ANCHOR supports the missions, vision and values of Northwest Community Care Network.

2. Coordinates and developments a vision, strategic plan and operational plan with which to operate ANCHOR.Leading the team to oversee the strategic plan and ensure a structure of appropriate members of organizations and the community. Collaborates with the ANCHOR Project Manager to integrate services and foster collaborative community relationships.

3. Is accountable for the assessment, planning, implementation and evaluation of ANCHOR services to program enrollees by assessing program enrollee needs in the community through various channels. Recommends, coordinates and collaborates regarding the development of programs to meet program enrollee needs based on assessments and impute from the community. Assists in the implementation and evaluation of the success of ANCHOR programs to ensure quality and efficiency, while avoiding duplication of internal and external efforts.

4. Collaborates with physicians and agencies to assure coordination of ANCHOR services, maintaining regular communication with those physicians and agencies regarding projects and programs to support transitions of care and enhanced program enrollee outreach. Also maintains regular contact with community leaders and organizations serving enrollees to enhance the effectiveness of program initiatives.

5. Is accountable for administrative operations, managing program budget, fiscal affairs, allocation of resources, and financial transactions while providing overall coordination of program development. Must ensure contract deliverables are met and reports inconsistencies to NCCN Deputy Director. Participates in the acquisition of new revenue streams to sustain ANCHOR. Supervises, hires, orients and mentors navigators.

6. Serves as liaison to various internal and external agencies and community groups by representing ANCHOR in activities directed toward the public, providing reports and presentations as appropriate and providing NCCN Deputy Director with regular progress reports.

7. Provides supervisory responsibility for key ANCHOR Program staff.
