The Transitional Care Advanced Practice Professional (APP) is responsible for coordinating the planning, implementation, and ongoing monitoring and evaluation of their designated patient population. As an Advanced Practice Professional, the APP is responsible for identifying and implementing opportunities to support and improve the quality of care provided to assigned patients across the continuum of care. The APP is a conduit for targeted patient populations between acute and primary care in the outpatient setting. The successful person in this position will be able to work well and learn together with a multidisciplinary team to see beyond diseases and co-morbidity to assess patients'' functional prognosis and adapt care plan to provide patient-centered and cost effective care. Candidates should feel comfortable practicing with significant autonomy within the scope of their training and in a variety of settings including; clinic visits, inpatient care, patient home visits, and partner facilities such as dialysis and skilled nursing facilities. This position also requires clinical flexibility and the ability to care for patients on a recovery trajectory as well as those requiring more palliative care.

Education Required: Master of Science in Nursing or completion of PA program
Licenses/Certificates Required: Certified Nurse Practitioner, BCLS or Certified PA
Years Experience Required: Two years of experience preferred. Expertise in field preferred.
