Job Description


This Department of Neurology is currently seeking and Nurse Practitioner/Physician Assistant to support the implementation of a Care Transitions Program for the General Inpatient Service in the Department of Neurology. The Advanced Practitioner will provide daily management of patients on the unit for half the day and will spend time as part of the rounding team. The remainder of time will be allocated to outpatient clinic activities, with the major objective involving seeing patients within 7 to 14 days of post discharge.
