Job Summary: The Care Management Program Coordinator will coordinate, direct, monitor, and evaluate activities regarding designated care management program areas. This Coordinator is responsible for teams of care managers who serve the local network region. The Coordinator ensures that the elements of the program are implemented in a standardize manner among their teams. The Coordinator must collaborate with local community agencies, health departments, hospitals, primary care practices, hospices, adult care homes, long-term care facilities, mental health agencies (LME/MCO), DSS, and state level organizations, including DMA and CCNC.

Education / Experience: Graduate from an accredited School of Nursing. Minimum of 3 years Case/Care Management experience. Minimum of 2 year supervisory experience.

Reports To: Manager Care Management, Northwest Community Care Network

Licensure, Certification and / or Registration: Currently licensed by North Carolina as a Registered Nurse. Valid North Carolina driver license. Certification in case/care management preferred.

Essential Functions:

1. Program Coordination: Will provide oversight and supervision to teams of care managers using reports based on administrative and care management data to ensure program measures, quality measures, and standardization are being met.

· Review and analyze data reports for care management activity, quality, and trends.

· Ensure priority reports, increased utilization volume, and unusual events are monitoredand reported to care management leadership team.

· Arrange and facilitate regular meetings with team supervisors to ensure complete understanding of program metrics, goals and updates.

· Collaborate with team supervisors and Network Quality Improvement Coordinators to review care management documentation, monthly activity, and ensure staff are meeting CCNC standardized plan. Will facilitate strategies to ensure program goals are being met.

· Communicate with care management leadership team on identified activity and quality trends.

· Will facilitate effective communication/collaboration with community partners to ensure coordinated and seamless care for the client. Participation in various workgroups to facilitate these collaborations as requested by Network Leadership

2. Quality improvement:Provide effective leadership in quality improvement activities and continuous improvement of care management activities.

· Identifies process improvement opportunities for care managers

· Effectively analyzes the care management processes using graphs, reports, and tools to present findings

· Implements and monitors strategies to ensure quality and efficiency in care manager work flow

· Will provide education and promotion of programs supervised to medical providers and health care providers in various settings.

· Works collaboratively with the Network's Quality Improvement team to coordinate the care of patients across systems of care by facilitating communication.

· Works with practices, when necessary, to facilitate the integration of care managers into the practice team

3. Workgroup Participation: The Care Management Program Coordinator will participate in local and/or statewide workgroups, as directed by department manager and will act as the liaison between NCCN and workgroup. Will communicate workgroup information to department manager and care management leadership team on a regular basis and identify areas that may impact care management activity, quality, or processes.

4. Human Resources: Will assist department manager in recruiting, selecting, and training new care management staff. Care Management Program Coordinator will be responsible for monitoring performance, evaluations, and staff development for their designated teams.

5. Education and Training: Attend educational meetings and trainings for continued education and adherence to NCCN, Community Care of North Carolina, hospital, and national accreditation organizational standards.

· Completes mandatory annual trainings and competencies

· Maintains a good understanding of the chronic disease process, evidence based treatment and management.

· Maintains a thorough understanding of case management standards of practice

· Maintains a good understanding of quality improvement processes

· Attends regional and local care management meetings as scheduled

· Attends continuing education programs related to initiatives and designated program areas

· Follows hospital policies and procedures

· Follows departmental policies and procedures

· Contributes to the overall quality of services

· Assumes responsibility for keeping informed about changes in policies and procedures

Job Summary: The Care Management Program Coordinator will plan and direct Network activities centered on Care Management processes and variability. This position will analyze data related to Care Management activities in order to develop strategies to improve adherence to the CCNC Standardized Plan and Network goals. The Care Management Program Coordinator will also partner with Care Manager Supervisors in the Network?s seven counties to improve work flow, monitor performance, and provide training and education on best practices to Care Managers.

Education / Experience: Graduate from an accredited School of Nursing. BSN Required. Minimum of 3 years Care Management experience.

Reports To: Senior Administrator, Northwest Community Care Network

Licensure, Certification and / or Registration: Currently licensed by North Carolina as a Registered Nurse. Valid North Carolina driver license.

1. Provides oversight of the Care Management workforce, using reports based on administrative and care management data to ensure program goals are being met
? Collaborates with Network Care Management Supervisor to set Network targets and review Care Manager performance

? Collaborates with Care Manager Supervisors in local agencies to set performance targets and ensure Care Managers follow the Community Care of North Carolina (CCNC) standardized plan
? Provides topic-oriented education to care managers regarding best practices

? Designs strategies to make improvements in work flow
? Develops plan to address Care Management variability across the seven county Network
3. Collaborates with practices, local agencies, and the Network Management Team to ensure effectiveness of care management
