Registered Nurse -Critical Care Unit of Wake Forest Baptist Health-Lexington Medical Center'sCritical Care unit is now seeking a week night experienced Registered Nurse for fulltime employment. The nurses partner with physicians from Wake Forest Baptist Health to provide high levels of quality, patient centered care. The member of this team oriented department will work the following schedule: weekdays from 7:00 P.M.-7:00 A.M. with a rotating weekend and holiday schedule. Minimum Requirements to be considered for this position include: 1 year of Nursing Experience, Graduate of an accredited School of Nursing, current RN license to practice in NC, and BLS certification.

1. Utilizes the Nursing Process to plan and implement in the delivery of patient care.

  • Initiates accurate and ongoing assessments of physical and psychosocial needs of patients within the department-specific guidelines as evidenced by documentation.
  • Collaborates with patient/significant other and members of multi-disciplinary team in the formulation of the overall plan of care.
  • Incorporates psychosocial, cultural, religious, and age-specific needs into the patient's plan of care.
  • Develops interventions appropriate for the patient.
  • Utilizes appropriate resources in providing patient care.
  • Initiates referrals as appropriate.
  • Evaluates and documents care based on patient responses.
  • Involves the parent(s) of infants and children in procedures, education, and planning.
  • Provides simple explanations to a child, tries to get them involved.
  • Encourages/ allows adolescents to have input and to ask questions.
  • Asks name patient prefers to be called.
  • Modifies environment as needed for the older adult. Increases lighting, positions to minimize glare, for comfort, reduces outside noise, etc…

2. Administers medication according to policy/procedure and standards.

  • Cross-references medication record with physician orders as appropriate.
  • Ascertains that the following is correct prior to administering medications:
  • the drug
  • the amount
  • the route
  • the patient
  • the time
  • Keeps medication cart locked when not in use.
  • Documents medication effectiveness in the progress notes and/or computer as appropriate.
  • Charts medications promptly after administration.
  • Follows up with physician and/or pharmacy on medications that deviate from prescribed use relative to normal range, age, or body weight, etc.
  • Uses a pediatric burette and IV pump when administering intravenous fluids.
  • Uses a dropper to give liquid medications while holding an infant in sitting or semi-sitting position.
  • Gains trust of a child. Explain to child when medication has undesirable taste.
  • Praises child for cooperation after medication administration. Observes for fear and/or stress, verbal and non-verbal behavior in the adolescent or child. Adapts approach to individual needs.
  • If known swallowing problems in an older adult, crushes uncoated tablets or mix medications in soft foods if not available in liquid form.

3. Teaches patient and family about conditions (if applicable).

  • Assesses patient for barriers to learning.
  • Answers questions regarding treatment plans.
  • Provides valid and accurate information.
  • Provides information at level of understanding.
  • Verifies understanding of information conveyed by the health care team.
  • Orients patient and family to staff, new environment, and routine expected for the department.
  • Keeps teaching sessions short for children.
  • Provides additional information, assess and corrects misconceptions for the adolescent.
  • Utilizes multi-faceted approach-verbal, written, demonstration in education and instruction for the older adult.

4. Demonstrates ability to perform CPR skills.

  • Successfully completes CPR educational training every year.
  • Initiates CPR according to hospital policy.
  • Ensures the Broslow Pediatric Bag is kept within easy access at the nurse's station.

5. Serves as Preceptor (if applicable)

  • Provides feedback to new preceptee and documents appropriately.
  • Completes orientation checklist.
  • Completes training requirements as appropriate.

6. Serves as Unit Charge Nurse (when applicable)

  • Completes Charge Nurse Report form at the start of each shift.
  • Completes Charge Nurse Communication record.
  • Performs narcotic count and ensures documentation is complete for accuracy, signatures, etc.
  • Performs/delegates stock medication count.
  • Performs/delegates daily crash cart checks.
  • Makes staffing assignment according patient acuity and departmental guidelines.
  • Documents attendance or absence of each employee on the attendance record.
  • Ensures all new admissions are entered into the computer and unit log.
  • Performs bed triage according to departmental guidelines.
  • Assigns dock, call, or pull for staff to other departments according to departmental guidelines.
