Inpatient/Emergency Transfers
To refer a patient for inpatient and/or emergency hospitalization, please call Wake On-Call staff at (336) 716-2255 or (800) 446-2255.
Continuity of Care
An open line of communication with the patient's referring and/or primary provider is essential to providing the patients with the highest quality of care. Upon discharge from the Medical Center and with the approval of the patients, the discharge summary will be forwarded to the referring/primary care physician for follow-up treatment and implementation of the patient's plan of care.
