Family Medicine – Piedmont Plaza I, an outpatient clinic of Wake Forest Baptist Medical Center, opened its doors in February 1992. Located on the southeast corner of the intersection of Stratford Road, Country Club Road, First Street and Miller Street (commonly known as Five Points), Family Medicine – Piedmont Plaza I is less than a mile from Wake Forest Baptist Medical Center. The clinic offers patients, staff and physicians the benefits of a community setting, while maintaining access to the Medical Center via a van shuttle service.
Four patient care modules, each with 10 examination rooms, a nursing station, a reception area, a waiting room, and consultation/dictation area, provide the backbone for the clinic. Learn more about our modules and our continuity of care. There is a large treatment room with a special sigmoidoscopy/colposcopy suite. Also available are x-ray facilities, laboratory and business office.
There are more than 50 providers at Family Medicine – Piedmont Plaza I, including faculty, residents and physician assistants. In addition, medical students from Wake Forest School of Medicine rotate through to learn about family medicine.

We take care of our patients while they are in the hospital. Depending on the nature and seriousness of the problem, you will receive care either from our Family Medicine inpatient team or from one of our subspecialty colleagues.

The Family Medicine Center is located on the 3rd floor of the Piedmont Plaza I building, which is in the southeast corner of the intersection of Stratford Road, Country Club Road, First Street and Miller Street (commonly known as Five Points).
Free parking is available in front of the building, with access from First Street or Miller Street.

Hours of Operation
Mon-Fri: 8 am – 5 pm
Sat: 9 am – noon (for current patients to be seen for urgent problems only)
When the office is closed, calling 336-716-4479 will connect you to our answering service. They will give your message to the on-call doctor who will call you back. Please be patient if this takes a while because the doctor is also caring for patients in the hospital and emergency room. If our on-call doctor recommends that you go to the Wake Forest Baptist Emergency Room, upon arrival let the nurse there know that you are a Family Medicine patient.
