Diagnostic Lab Request Forms
To order Anatomic Pathology tests (including cytology, dermatopathology, and surgical pathology) from a clinic, hospital, or laboratory outside the Medical Center, use the appropriate form below. You can complete them onscreen before printing, and you may email a copy to the appropriate physician. All specimens must arrive with a completed, signed request form.
