Memory Assessment Clinic
The Memory Assessment Clinic provides high quality, compassionate care to older adults who are concerned about changes in their memory. The Roena B. Kulynych Center Memory Assessment Clinic of Wake Forest Baptist Health is one of the few memory assessment clinics in the nation run by specialists in geriatric medicine. We are staffed by a team of specialists in the care of patients with memory related diagnoses including geriatric medicine faculty and fellows, counselors, and neuropsychological evaluators who put together a treatment plan that will address the whole person and his/her family, not just the disease. For example, we integrate counselors into our assessment to address the psychological needs of our patients and their families as they struggle with memory loss. The counseling program offers both individual counseling and support groups to our patients and their families.
Who We Serve
The Kulynych Center Memory Assessment Clinic provides comprehensive assessments to older adults who are concerned about their memory or who may already have a diagnosis of a memory problem and are seeking a second opinion.
What We Do
We specialize in the diagnosis and treatment of memory related conditions such as mild cognitive impairment, Alzheimer's disease, Lewy body dementia, vascular dementia and frontotemporal dementia among others.
The initial evaluation takes about 3 hours and includes:
- A comprehensive history (interview) of both the patient and someone who knows the patient well
- A complete physical examination
- Review of any previous records, labs, and scans
- A thorough review of all medications (including over the counter medications)
- 1 hour of cognitive testing
- If necessary, blood work and a brain scan will be ordered
After all the information is gathered and reviewed, the team discusses the patient and arrives at a diagnosis and plan for further evaluation if needed or a treatment plan if no further information is needed. The doctors then discuss the plan with the patient and family. We will also send a letter to the patient's primary care physician describing our diagnosis and recommendations.

For general information about the clinic or to refer a patient, please call our clinic coordinator at: 336-713-8963. Appointments are scheduled on Tuesdays from 8:30 am to 5:00 pm. We prefer to have a referral from the patient's primary care doctor.
Location
The Kulynych Memory Assessment Clinic is located on the ground floor of the Sticht Center on Aging. Patients should check into the clinic through the Geriatrics Outpatient Clinic, also located on the ground floor.
