Please visit the Office of the Registrar to select the appropriate method for submission of your request based upon your specific needs.

For the Biomedical Sciences degree programs (PhD, MS) provided through the Wake Forest Graduate School of Arts and Sciences, please visit the Registrar’s Office online for more information.

For the Nurse Anesthesia Program, please send an email to napinfo@wakehealth.edu that includes the following information: name while in the program, year graduated, name and address of facility to forward a copy of the transcript.

For our Pastoral Care programs, please provide a written request (noting your full name during your time as a student) for a letter of verification of completed courses or evaluations, and mail to:
