Submission Forms

• PTO/Time Change Request (*Note, do not submit a PTO request that does not comply with the PTO rules published in the student handbook).
• Clinical Conference submission form

 Fill out sections "SRNA" and below. Save for your records and email to Courtney. You must submit a case once each semester. Keep all information about the case (copy of anes. record, etc.) in your possession and accessible. If you are unable to access the form, you may submit the case as a Word file instead. Include MD, CRNA, SRNA, Date, and synopsis of case of about 400 words or 3/4 length of typed page.
