How to Participate
Women Junior faculty wishing to become a mentee - please complete the Mentee Needs Assessment form (online version) (paper version)
Application Forms should be returned to Diana Cornelison, the Mentoring Program Manager at firstname.lastname@example.org, for purpose of identifying a Career Development mentor(s).
For senior faculty wishing to become mentors, enrollment is a two-step process:
- Interested senior faculty should fill out a Mentor Application form (online version) (paper version)
- Following acceptance, senior faculty will be requested to fill out a Mentor Areas of Interest Form
