Incontinence Impact Questionnaire & Urogenital Distress Inventory
Now Available: Survey Instruments to Assess Impact of Urinary Incontinence on Women's Lives
Office of Women in Medicine and Science (OWIMS) {formerly the Women’s Health Center of Excellence for Research, Leadership, Education} researchers developed and validated two survey instruments for collecting detailed data on how urinary incontinence affects women's lives.
The Incontinence Impact Questionnaire (IIQ) measures the impact of urinary incontinence on activities, roles, and emotional states. The Urogenital Distress Inventory (UDI) measures how troubling the symptoms are to women.

OWIMS administers the distribution and use of these two questionnaires. On request, we will send copies of the self-administered instruments (both short and long forms), and scoring materials for each instrument.
There is a one-time, $250 administrative fee for use of these questionnaires.
