If paying by check or money order, please make tuition check payable to, "Department of Radiation Oncology - Wake Forest University Health Sciences," including the name of participant and last five digits of the participant's social security number.
Please note: All cancellations must be received two months prior to start of course or tuition will be applied to a future training course. If at least six participants have not registered for a course date within 2 months prior to the course then we reserve the option to cancel that course date with full tuition re-imbursement. Two months of lead time is needed to fairly allow participants to make appropriate travel and clinical coverage arrangements.
Course tuition will include course fees, instructional materials, and meals (breakfast/lunch Day 1 and 2) and dinner with faculty on evening of Day 1.
