Rotating Residents

All residents coming to Wake Forest Baptist Medical Center to do a rotation must provide the House Staff Office with the following:

  • Written documentation, signed by the program director of the resident’s sponsoring institution, stating that said institution is continuing to cover the rotating resident’s salary, benefits and malpractice insurance during rotation
  • Verification of malpractice insurance, e.g. certificate of insurance
  • A valid N.C. residency training license

A residency training license can be a time consuming process. We suggest that you obtain the application and begin the process as soon as you determine that you will be rotating here. You must allow at least 1 month for this process. If a rotating resident does not have a residency training license prior to the time of rotation, he/she cannot perform hands-on patient care, therefore the rotation would not be beneficial.

Download the North Carolina Residency Training License.

Please read the instructions for the application very carefully. Anything not completed properly will delay the application process.

Note: If you have already graduated from medical school, your school must complete Medical School Certification Form B. Form A is only for those who have not graduated. The Board will not accept the wrong form.

If you have been certified or licensed in any medical field, such as EMT, nurse, certified nursing assistant, etc., you must have either the verification of license or certification form in the application completed by that licensing or certifying board. A training license cannot be issued without that documentation.

The application must be forwarded to the WFBMC House Staff Office, Attention: Diane Hartley. Do not send the application directly to the Board. If you have questions or concerns about the application or the application process, please contact us at

In addition, please review the Non Employee Personnel Policy.

Thank you for your interest in one of our training programs.
