How To Download Your Medical History

To view information on how to download your medical history, please follow the instructions below: 

IQHealth                    Relay Health


If you have not already signed up for myWakeHealth, you can join now.

Relay Health 

Downloading Care Document Summaries: 

1. Click on the Health Record Tab (located in a gray bar at the top of the screen) 
Click Health Records

2. When the drop down menu appears, click documents

3. Choose documents by their service date.  Click the drop down arrow located next to service date
Choose documents by their service line

4. Click Download

5. Choose PDF as the file format, then click the blue download button
Choose PDF

6. A PDF file will be created and can be saved to your computer

Printing Labs or Radiology Results: 

1. Click Health Records
Click Health Records

2. Click Result Type (drop down arrow located to the far right) 
Click Result type

3. Select View Result

4. Right click on your screen and select print 

5. Choose the print option from your computer


IQHealth 

1. Log in 

2. Go to your Health Record

3. Select Documents on the left side of your screen

4. Select Download

You may also download your history via your after visit summaries: 

1. Go to your Health Record

2. Select Visit Summaries on the left side of your screen

3. Sort by time frame

4. Select Download for each summary
