Job Summary: The Diabetes Educator demonstrates mastery in the body of diabetes knowledge and treatment therapies while utilizing behavioral and case management interventions in the care of patients with diabetes. The Diabetes Educator provides quality diabetes self-management education (DSME) to all referred patients with diabetes and their families through the implementation of the American Diabetes Association (ADA) Recognition standards of care. The Diabetes educator works with a multifaceted education instructional team using their individual disciplinary expertise to collaboratively provide comprehensive age appropriate diabetes education. Management responsibilities include intensive therapies which require critical thinking and independent judgment in collaboration with the patient, referring physician, medical director, and other designated members of the health care team. The Diabetes Educator maintains extensive follow-up for complex patient population while exploring pertinent resources for improved outcomes.
Education / Experience: Graduation from an accredited school of nursing-BSN preferred, or graduation from an accredited school with a preferred Baccalaureate Degree in Human Nutrition, Foods and Nutrition, Biological Sciences, Dietetics, Sociology, Public Health, Psychology, or Pharmacy. Must have broad ranges of knowledge on diabetes for age appropriate population and its interrelation to co-existing health problems. Minimum of two years of diabetes related work experience. Patient oriented educational experience preferred.
Reports To: Center Administrator
Licensure, Certification and / or Registration: Current licensure or certification to practice as Registered Nurse in North Carolina, Registered Dietitian, Registered Pharmacists, Social worker, Family Counselor, Health Educator, or Exercise Physiologist. Certified Diabetes Educator (C.D.E.) preferred. If not certified, must obtain certification within two years.
- Maintains competency skills by participating in continued education and professional development.
- Provides patient management by assessing, planning and documenting.
- Implements interventions as per dept. guidelines, management protocols, and specific patient needs.
- Evaluates patient's progress and revises plan of care as needed.
- Demonstrates proficiency in all aspects of diabetes education.
Skills and Qualifications:
- Good communication skills including verbal, non-verbal, and listening
- Must have excellent interpersonal skills and working knowledge or principles of human learning
- Good organizational skills
- Clean, well lit, comfortable
- May be exposed to infections and contagious diseases
- May be exposed to/occasionally exposed to patient illnesses
- Occasionally subjected to irregular hours
