Midlevel Providers Description of Duties
IN THE PREOPERATIVE ASSESSMENT CLINIC
After a predefined training period, provide patient care services by independently and interdependently performing history and physical examinations and preoperative risk assessments on patients scheduled for surgery. The physician''s assistant or nurse practitioner is expected to independently participate in the delivery of direct patient care as it applies to the Preoperative Assessment Clinic, review all ordered labs, ancillary study results, and information provided from the patient''s primary care physician. Perform complete history and physical examination prior to a variety of surgical procedures, formulate a plan as it relates to the patient''s comorbidities and potential anesthetic, and review with anesthesia faculty present in clinic, as needed. Follow patients through the preoperative period to ensure all details are addressed and patient is medically optimized to minimize postoperative risks.
Coordinate and schedule preop laboratory studies, diagnostic procedures, and preoperative consultations with specialists as needed. The midlevel will coordinate and communicate with the PAC attending regarding the results of studies that may change the course of surgery or anesthetic and arrange for follow-up if cancellation of surgery necessary.
Communicate directly with anesthesiologists and surgeons regarding the results and recommendations arising from the preoperative assessment through the use of the EMR applications. Assist in determining optimal timing for surgery and medically optimizing patients prior to surgery. It will be a critical duty to carefully identify and minimize patients with risk factors for perioperative complications and discuss final disposition and concerning findings with the preoperative assessment clinic attending.
Familiarity with computer systems for the acquisition of patient information from outside sources and remaining current with evidenced based medicine to minimize risk factors and optimize patients preoperatively will be necessary. Maintaining competency standards as set forth by the medical director of the preoperative assessment clinic will be required. Adaptability in terms of assessing a variety patients (Pediatric and Adult) will be required.
Variable rotations are available within the Section on Perioperative Medicine. Our midlevel providers have the option of conducting inpatient (hospital) assessments and/or clinic-based assessments. Our inpatient preoperative service functions similar to an inpatient hospitalist preoperative service.
