Job Summary:The Adult Transitional and Supportive Care Manager provides clinical and administrative leadership for the Adult Transitional and Supportive Care Programs through experience and education in end-of-life care, program administration, and research-related activities. The Care Manager is responsible for coordinating and facilitating all supportive care activities and operations as they relate to physicians, advanced practice providers, and supportive care resource nurses within the adult population of Wake Forest Baptist Health. This role will manage the supportive care resources nurses and enhanced care nurses; maintaining responsibility for their ability to provide patient-centered care across the continuum. Works in collaboration with the Medical Director of the Adult Transitional and Supportive Care Program and Care Transitions leadership to plan, develop, coordinate, monitor and evaluate operations of the adult Supportive Care Program; including all physicians, advanced practice practitioner and nursing work. Has 24-hour accountability for the quality, patient and workplace experience to establish and maintain an effective care and workplace environment. The Care Manager will establish unified, cohesive operations between an interdisciplinary team consisting of physicians, nurse practitioners, nurses, social work, and chaplains.

Education / Experience:Graduate from an accredited school of nursing, MSN, required, Advanced Practice Practitioner preferred. CHPNA or LNEC certification required within one year of hire. Previous experience in hospice, palliative care, transitional care, or other end-of-life areas required. Minimum of 5 years experience in nursing.

Reports To: Director of Nursing and Care Transitions Administrator

Licensure, Certification and / or Registration:RN licensure in NC. Basic Cardiac Life Support (BCLS) required. Advanced Cardiac Life Support (ACLS) may be required within one year of hire. Palliative Care Nursing Certification or relevant nursing certification required within one year of hire.

Essential Functions

1. Manages the identification of patients and families who may benefit from transitional or supportive care provider and resource nurse consultation.

2. Works in collaboration with supportive and care transitions leadership on initiatives related to the program.

3. Acts as a patient advocate throughout the continuum of care including all patient settings and in the community. Coordinates the effective and efficient delivery of patient care services through the application of care management principles. Identifies clinical priorities and collaborates with team to initiate actions to improve patient outcomes. Educates nurses, house staff, and other medical service teams on both Care Transitions and SupportiveCare processes.

4. Demonstrates ability to function in a professional practice model through active participation in shared governance, acceptance of personal responsibility for professional growth, exhibiting critical thinking behaviors, and demonstrating inter-disciplinary team leadership and clinical excellence. Demonstrates responsibility for personal development by participating in continuing education offerings, and develops best practices to maintain the highest level of patient care and staff development.

5. Creates integrative processes that ensure appropriate patient placement in the appropriate level of care.

6. Coordinates care processes with inpatient, outpatient and community services.

7. Provides leadership to assure appropriate orientation, education, credentialing, and continued professional development for personnel.

8. Collaborates effectively throughout the organization by developing collegial relationships and communicating information effectively, encouraging communication and serving as a resource to staff and other departments.

9. Has responsibility with the supportive care medical director and care transitions leadership for planning, development, implementation, and monitoring of the budgetary process for assigned areas. Assures personnel and resources are allocated based on national bench marks, quality indicators and diverse patient needs. Facilitates and monitors the appropriate utilization of staff based on credentialing, education and experience.
