Work as a member of a Patient Care Team to provide pharmaceutical care to adult patients. Activities will include but are not limited to rounding with a team, attending unit huddles, optimizing patients' drug therapies, medication reconciliation, offering discharge counseling, utilizing the medical literature to assist in determining therapeutic treatment recommendations, and assisting in continuity of care. In the pharmacy, this pharmacist will facilitate efficient drug distribution and preparation of medications to patients served. This pharmacist will assist in managing the activities of a team of pharmacy technicians, verify orders, serve as a drug information resource for patients, nurses and medical staff, and maintain skills and knowledge by attending required educational in-services and skills development classes. This individual will be cross-trained to other rounding services and other pharmacy areas as needed. This pharmacist will take the annual Pharmaceutical Care Exam. This pharmacist will also participate in quality improvement and strategic planning activities which promote optimal patient care, teamwork, and NCBH's shared key values.
