About Me

“Man did not weave the web of life-he is merely a strand in it. Whatever he does to the web, he does to himself… (Chief Seattle)”. I am Leah C. Brown and I believe the power of these words. I am the daughter of Rev. Dr. Leamon E. Brown and Mrs. Beatrice G. Brown of Salisbury, NC. I graduated early as an honors student from Salisbury High School and am a proud, Magna Cum Laude alumna from Winston Salem State University. I earned a Master degree from the UNC-Chapel Hill School of Social Work. 

I have nearly 20 years of Social Work experience. Most of my time has been working as the Cardiac Transplant/LVAD social worker and discharge planner for patients coping with CHF. Currently, I am a Transitional/Supportive Care Navigator in Population Health. My professional role aligns strongly with advocacy for the disenfranchised and enables me opportunities to coordinate services that prevent unnecessary hospitalizations and promote social stability.  

I continue to work with the Advanced Heart Failure clinic as a resource and am presently working on a project with other community social workers to address the needs of dialysis patients in Forsyth County.
