Welcome to Wake Forest Baptist Health and the exemplary training journey we
are commencing together. Please review the following checklist for required
on-boarding activities. We encourage you to start as soon as possible.
A) First Things First (1 ONLINE FORM)
Fill out the New
House Staff Information Form Online
. This provides us with a chronology of your prior training and gives you
the opportunity to explain any training gaps.
Please provide GME with a current address and your personal email address -
gmail, yahoo, hotmail, etc.
: The New House Staff Information Form must be submitted 90 days before your start date.
B) 1 email (Background Check)

to kick-off the background check process.
C) Resident Training License (RTL) (ONLINE & MAIL)
Complete the online
Resident Training License (RTL) Application
(North Carolina Medical Board). Follow the
NC Board RTL Checklist
to help with the process to obtain your RTL. The cost is $138 payable by
credit card only.
Mail the required GME documents, listed on the RTL checklist to the GME
: This process takes time and you must receive your RTL before you may begin training.
D) 1 link (To report American Heart Association
Certification, or sign up for recertification)
Your American Heart Association Certification must be current before your
first clinical day. Please check the list of requirements for your specialty
and follow the directions for completing the House Staff form.
E) 1 email (from a Wake Forest Talent Coordinator)
You will receive an email from a talent coordinator asking for information
regarding for important on-boarding forms to be completed before your start
F) Upper-level Residents and Fellows (1 E-MAIL)
Please notify the House Staff Office if you are applying for a Full N.C.
License. The ACGME only requires a RTL. Please check with your individual
program to determine if the program requires a full license. This involved
process may equate to 5-6 moths. Please start early with the
North Carolina Medical Board
. Once you receive your Full License, please email a copy to the House
Obtaining a full license for moonlighting purposes also requires you to
apply for an independent DEA number. The DEA number assigned to you from
The Medical Center is exclusively for training purposes.
Residency training and full licenses renew on your birthday regardless of
the date it was issued.

Please be advised that you will need to bring copies of your immunization

H) I-9 Verification of Employment Eligibility (1 IN-PERSON VISIT
You will be required to have your I-9 (Verification of Employment
eligibility in the United States) completed prior to orientation. You must
present supporting documentation in person for
verification and you will fill out the I-9 form at that time. Your
documents must be original and not expired:

All House Staff are required to have a documented NPI before commencing

specialty taxonomy code. Applicants who are applying for an NPI under a
Residency Training License must use the student
