Please note that you will be charged a $140 consult fee at the time of your visit to cover the cost of the audiologist services. The consult fee will be deducted from the cost of a hearing aid purchase, should you chose to purchase a hearing aid.

If you have flu-like symptoms or have been exposed to someone who has been diagnosed with, or suspected to have, COVID-19, please call 336-716-3103 and we will help you make an appointment.


Please confirm the following:

 Patient is new to this provider.

 Patient has not had a hearing test within the last 6 months.

 Patient is age 18 or older.

 Patient does not have flu-like symptoms and has not been exposed to someone who has been diagnosed with, or suspected to have, COVID-19.

 Visit is being requested for a hearing evaluation/hearing aid consultation.

 Patient does not currently wear hearing aids.
