Please confirm the following:

 Patient is requesting an appointment for COSMETIC services only.

 Patient is new to this provider.

 Patient is 18 years or older.

 Patient acknowledges awareness payment is due on date of service. If consult only, there is a $75 consult fee.

 Patient does not have flu-like symptoms and has not been exposed to someone who has been diagnosed with, or suspected to have, COVID-19.
