 Patient is new to this provider.

 Patient is age 18 or older.

 Patient does not have flu-like symptoms and has not been exposed to someone who has been diagnosed with, or suspected to have, COVID-19.

 Patient has private insurance, Medicare or Medicaid. (If not, please call the number above and we'll help you schedule.)

 Patient acknowledges that if unable to provide insurance coverage verification at the time of visit, they may be asked to pay a $100 deposit towards the initial visit prior to being seen.

 Patient acknowledges Wake Forest Baptist Pain Centers primarily practice intervention-based medicine which may include procedures, physical therapy and limited medication management to treat chronic pain conditions.

 Patient acknowledges no opioid medications will be prescribed during initial visit.
