Whenever you’re faced with something you don’t understand – whether it is a diagnosis, a medical procedure, or simply directions to the cafeteria – please ask your physician, nurse, or any other hospital staff member for further explanation.

Advanced care directives are specific instructions, prepared in advance, that are intended to direct a person's medical care if he or she becomes unable to do so in the future.
For more information please take a look at a brochure developed by Wake Forest Baptist Health for patients that explains what advance directives are: Advance Directives: Planning for your Future Health Care. We also have an education sheet, It's as simple as 1,2,3, for patients from gotplansnc.org that provides a checklist for patients interested in the advance care planning process. Lastly, please see this education document for patients developed by the NC Division of Medical Assistance explaining advance directives.
If you need additional assistance with any problem, concern or to file a grievance our Office of Patient and Family Relations is ready to help you at 336-713-2273. Making a complaint or grievance will in no way compromise your future access to care. You may also lodge a grievance directly with:
