Executive Team

Steven Smith, MBA

Steven is the Executive Director of Care at Home. He graduated from the University of Florida with a degree in Occupational Therapy and received his master’s from Gardner Webb University. He enjoys traveling to watch soccer, football and basketball games.

Kristen Craver, RN

Kristen is Director of Clinical Management for Care at Home and also leads the transitional care and cardiopulmonary teams. She is a graduate of Surry Community College. She is a wife and mother of three children. She keeps a turtle in her office named “Speedy,” who is the unofficial mascot of Care at Home.

Nursing

Melissa Allman, RN

Melissa is the Quality Assurance Manager of Care at Home. Melissa graduated from Winston-Salem State University. She has five children. In her spare time, she enjoys biking and traveling to the coast.

Pamela Brady, RN

Pamela received her nursing degree from Winston-Salem State University and has 34 years of nursing experience. She is a native of Winston Salem. She has two adult children, and enjoys cooking and spending time with her family.

Tiffany Culler, RN

Tiffany is Manager of Clinical Practice for Care at Home. Tiffany received her nursing degree from Winston-Salem State University. She has 17 years of experience in nursing. She is a native of Cherry Point, N.C., and currently lives in Mount Airy. She is married and has two daughters. Her interests include hiking, walking and attending her girls’ sporting events.

Toyoka Davis, RN

Toyoka is a native of Winston-Salem and received her nursing degree from Forsyth Technical Community College. She has seven years of experience in nursing. She is married and has one child. She enjoys dining out, shopping and spending time with family.

Sally Lewis McCusker, BSN

Sally received her bachelor’s degree in nursing from the University of North Carolina at Chapel Hill. Before coming to Care at Home, she was a Clinical Nurse Educator and Quality Nurse Consultant at Aetna. Sally also has experience in Med/Surg Nursing, Public Health, Pharmaceutical Medical Affairs, and in the conduct and oversight of Phase 4 Clinical Trials. Sally, and her Havanese, Pippa, live in High Point, where they enjoy road biking, watching SEC football (Roll Tide) and PGA golf.

Rebecca Ogden, RN

Rebecca received her nursing degree from Massachusetts Bay Community College and has more than 14 years of home care nursing experience. Becky has three granddaughters, and she enjoys playing with her grandchildren and dogs, reading and traveling.

Malynda Talbert, LPN

Malynda received her LPN degree from Forsyth Technical Community College. She has 26 years of experience, with the last nine in home care. She is married and has two adult daughters. She enjoys gardening and doing crossword puzzles.

Therapy

Shari Collins, PTA

Shari received her degree from Delaware Technical Community College. She has 10 years of experience in Physical Therapy. She lives in High Point, and enjoys aquatic activities, and spending time with her family and friends.

Melissa Elrod, OT

Melissa received her degree from West Virginia University. She has 10 years of experience in Occupational Therapy. She has a twin sister who is also an Occupational Therapist. Melissa lives in High Point with her husband and son. She enjoys music and going to concerts.

Khristine Javelosa, PT

Khristine is from the Philippines and received her bachelor’s degree from St. Paul University Iloilo. She has worked since then for an orthopaedic specialty clinic outpatient facility and doing home care services. She moved to the United States in January 2009 and began working in home care in Florida for several agencies before relocating to North Carolina in July 2012. She began working with Gentiva Home Healthcare the same month. Her husband is a CNA, and she has 2½-year-old twin boys.

Carol Mark, SLP

Carol received her degree from the University of Wisconsin, and also has a master's in Business Management from Cardinal Stritch University. She has more than 30 years of experience in Speech Language Pathology, with a background in Long Term Care, Health Care Operations and Home Health. Carol and her husband live in Pfafftown and have two grown daughters and one grandchild with another on the way.

Kristie Parker, PT

Kristie is Rehab Team Lead for Care at Home. She received her degree from the University of North Carolina at Chapel Hill. She has nine years of experience in Physical Therapy. She lives in Clemmons with her husband and two children. She enjoys playing with her children, traveling and church activities.

Lauren Williamson, PT

Lauren graduated from Shenandoah University. She is married and has two children. She enjoys traveling and loves visiting Europe.
