Outpatient clinical nutrition therapy involves the assessment of nutritional needs of neonatal, pediatric, adolescent, adult and geriatric age groups and culturally diverse populations based on disease specific needs so that medical care is optimized. Based on this assessment, our clinical nutritionist will develop an individualized plan of care which will include treatment goals.

Consultation with your referring and primary care physicians is integral to developing a comprehensive nutritional plan of care.

Outpatient clinical nutrition therapy includes, but is not limited to, treating the following conditions:

  • Amyotrophic lateral sclerosis and other neurological disorders

  • Celiac disease and other GI disorders
  • Crohn's and Colitis (IBD)

Our registered dietitian (RD) provides nutrition education and goal setting and teaches skills to patients, their families and caregivers in order to optimize the patient’s nutritional status and ultimately improve their medical condition.

Referrals and communication with in-home health agencies, the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), as well as other community agencies, is available to assure continuity of care.

What to Expect at Your Appointment

Our nutritionists will gather medical history to assess your current nutrition status using the methods of food frequency and 24-hour recall. Once a complete history is acquired, an individualized diet/meal plan will be created for you, and handouts regarding this will be provided and discussed.

An appointment can be scheduled by asking your primary care physician for a referral to outpatient clinical nutrition. We will bill your insurance company, Medicaid or Medicare for allowable services.
