We will guide you through a very efficient process to get you started with your UnitedHealth Group weight loss benefit through Atrium Health Wake Forest Baptist Weight Management Center. Please follow the simple enrollment steps outlined below.
Step 1:
Complete your online registration form.
Your enrollment process starts with completion of the registration form. Once complete, we will guide you through the rest of the online enrollment process. Those remaining enrollment steps are shown below (steps 2-4).
Step 2:
Watch the Program Information Video.
This is a crucial step in the enrollment process. You will watch a 30-minute video that will overview our programs and help you determine which program might be best for you. You will be able to watch this video as soon as you complete your online registration form (above).
Step 3:
Complete your New Patient Enrollment Forms.
You will be able to complete these forms securely online as soon as you watch the Program Information Video.
Step 4:
Schedule your Orientation and Scheduling Call with a Weight Management Center teammate.
This 30-minute call will allow us to review your information, answer any questions you might have, and set up appointments to start your program. We will take you to this step as soon as you complete your New Patient Enrollment Forms.
